We are committed to providing quality community based health care services to individuals eighteen and over who suffer from chronic mental and physical impairment including Alzheimer’s Disease and other forms of Dementia, Down’s Syndrome, Cerebral Palsy, Mental retardation, Brain injury, Stroke survivor, Schizophrenia, Diabetes, Chronic hypertension, and/or those who are in need of rehabilitation services to maintain or improve their level of functioning.
Our participants are at least 18 year of age who suffer from chronic physical or mental impairments including Alzheimer’s Disease or any form of dementia, Down’s Syndrome, Cerebral Palsy, Mental retardation, Brain injury, Stroke survivor, Schizophrenia, Diabetes, Chronic hypertension, and/or those who are in need of rehabilitation services to maintain or improve their level of functioning.
To participate in the program, a person can either be referred by a physician, family members, friends, caregivers, social workers, or even by themselves. Contact us to get started.

Our services are covered by: Medi-Cal, SCAN, Regional Center, or Private Pay
